Continue tabbing through the dialog. One tab will recite the highlighted text, the next will give you the option of anchoring to that text. Press spacebar to accept. Then tab to Okay and once again press the spacebar.
As for when to use this option, I can't think of an instance when I haven't. In theory, I suppose you could select a place on a webpage where the text changes, depending on context. However, I've never set a placemarker at such a location.
