Ok so before the remove still was associated with a thread and the pool
wouldn't take the instance.  In fact it seems that the pools never reclaimed
the tx'ed context on remove (;-)... since we were going quite high in the
loads it does prove that a stable heap at 6M is OK for most VM on LInux and
NT... ok now the stuff is done properly and the ctx association with is
dealt with properly.
