On Friday, the web server (dbForms.org) was down and I did not manage to finish copying the distribution package. It should be available in a couple of hours.
Note: http://www.dbforms.org/dist/dbforms_v1_1_1_20020505.zip is a typo - it is dbforms_v1_1_1_20020503.zip
