you already have said within the thread that under certain conditions the
cookie is created - so the following words might not apply to *your*
specific test environment :

Make sure that the used browser(s) allow(s) cookies - especially that you
"catch" the case that the final user of your page(s) has switched that
option OFF.
Perhaps the above line is the most stupid answer of the week - MSAW - isn't
it ;-)

Nevertheless - If you want graphically enhanced solutions - and if you want
to stay with your (and/or the list's) better knowledge of Java (instead of
JavaScript) :
You can "quite easily" mix input buttons and Java applets - see e.g.
http://uuhome.de/penski/soccer.htm or http://penski.tripod.com/soccer.htm as
most visited
example (from several others of my website).
