Yes, you have to create all that.

Click on the "Fields" link in the Interface Type screen to be able to add
fields.
counter_id should be of type Index.

PS. I see that there is snmp_walk() function used in JFFNMS. It looks
like equivalent to PHP snmpwalk(). I need snmpwalkoid() which doesn't
have JFFNMS equivalent. Can I use it without any problems?
Yes, its integrated into snmp_walk (check the $include_oid parameter). You
should ONLY use snmp_* functions and not the internal php functions as the
jffnms ones provide multiple SNMP version support and other stuff.
