You need to have jaws installed on both the client and server (although the server does not need a full install, there is an option you can pass to the installer for this but I forget what it is), I believe the server does need speech turned on (although I admit this doesn't make a lot of sense because jaws doesn't generate any sound on the server under this setup).
The most useful keystroke to know is "CTRL+ALT+BREAKĀ " which toggles the RDC client between full screen and windowed mode. Other keys are listed at: http://msdn.microsoft.com/en-us/library/aa383500%28v=vs.85%29.aspx Several of the listed keystrokes (like alt+page up/alt+page down allow you to perform the listed action when the client is in windowed mode, but depending on the options you have selected in the RDC client you can use the normal keystroke when RDC is in fullscreen mode.
