This is usually due to the sender's mail software. Some mail software
only provides "reply to sender" and "reply to all" options, in which
case the sender chooses "reply to all" - one copy is sent to the author
of the original message, and another to the mailing list, which in turn
sends it to the subscriber who happens to have written the original
message. Consequently, the original message's author receives two copies
of the reply: one from the sender of the reply directly, and another via
the list.
Better mail programs offer a "reply to list" option that sends the reply
only to the address of the mailing list.
