Brad, This won't help, but will affirm...I tried to set up a survey using Survey Monkey a couple years ago for a college course, and ended up having a work colleague do it for me. It was very *not* accessible to set up the survey.
Yes, in spite of it being easy to use to complete a survey, creating one is a very different matter. I don't know of any alternative, sorry.
