Just tried 2.0.1, and it works for me using the example you quoted.
The HTTP Request is created with two Parameters as expected.

This was using the following test plan:

Test Plan
+ Thread Group
+ + Cookie Manager
+ WorkBench
+ + HTTP Proxy Server 
 (Capture Headers, Set KeepAlive)
 (Target Controller: Test Plan > Thread Group)
 (Grouping: Do not Group Samplers)

I suggest you check the jmeter.log file and console output for any
error messages.
