This has been discussed on the list recently. Set up a counter variable, and use the variable to construct the name and password.
Or use your favourite scripting language to create the file for use by CSV data set. You are unlikely to be able to use a single instance of JMeter to run more than 200-300 threads at the same time, so you may need to run multiple instances in parallel.
