Dear Nico,

We are using http://www.mediawiki.org/wiki/Extension:ConfirmAccount almost 
since the beginning of Proteopedia, and are quite happy with it.

The extension has a nice user interface for requesting accounts, and an easy to 
use administration interface for approving, rejecting or place on hold requests.

The installation is quite straightforward, but I'll be happy in answering 
questions should you have any.

They do not insert much spam thanks to the measures you activated, but
having this number of useless entries will eventually hamper the system.
In my opinion you should disable user auto-sign up altogether. I don't mind to
take care of accepting new users manually; it will be less work than clearing
all these fake entries.
