As Steve said, it is a microphone. There is a small B&W version

This scratchy distortion must be on the master tapes, as it
has appeared on every version of "Blue"
There are several examples of it on the album.
I used to have a list of them all, but can't find it. There are 6 or 10
of them on various songs. My guess is that they were caused by
a faulty mic preamp. They either went unnoticed, or they knew they
were there, but the performances were so powerful they
just decided to leave them in.
