Besides these two, the precision will be influcenced by the resolution of your monitor (because the mouse cursor position is reported in full pixels) and by existing data and various configuration options (most importantly, mappaint.node.snap-distance). The default for that is 10, so clicking within 10 pixels of an existing node or way will actually place your new node on the existing object. As you correctly say, these 10 pixels may mean 10 metres or 1000 kilometres depending on your zoom level. I just clicked on Paris and a node was placed in Karlsruhe (becuase I had only loaded some data in Karlsruhe and was zoomed out that far...)
