i wonder if the problem is not due to your directory structure: your select_forms.html page resides in a directory, so it loads formularis_xml.php in the same folder.
but if you load that select_form page via ajax, the root becomes the loading page, so not the xmls folder. try putting everyting in the same folder and see if that solves the issue
