It "should" be possible. At least if you have a NTv2 grid to define your transformation.

You should try to define your transformation with a +nadgrids=grid_name.gsb parameter
as in the IGNF registry and to add your transformation in the epsg file.

Not sure it will be so easy, but have a try and let me know how it goes.
