Each time you want to open a file from dolphin a new instance is created.
Closing kate using the close button of window frame doesn't stop current
instance. Therefore you have multiple instances running in the background. I'm
not sure which instance is selected if you try to open the next file from
dolphin. If you force closing the "background" instances it works as expected.
Using File->Exit works.
