The database includes Disk ID's which won't match and of course include records to files that don't exist. I think we've pinned down the issue pretty well. You can stop Baloo, rename the ~/.local/share/baloo/index file, restart and reindex from scratch. My guess is that you got to a 'weird state', wierd enough to be caught by an assert, and you should get back to normal if you reindex.
