I work in a shared office, so I always want my sound output through my headphones, never through my computer's internal speaker. I go to System Settings->Multimedia->Audio and Video->Audio Hardware Setup->Device Configuration->Connector, change that settings from "Speakers" to "Headphones", and hit "Apply". This works perfectly. The problem is that the setting applies to the current session only; I have to re-apply it every time I log in. There is apparently a kludgy workaround (which I have not tested myself) described at <https://forums.opensuse.org/showthread.php/492740-KDE-How-to-set-the-Phonon-for-permanent-Headphones?p=2603358#post2603358>.
