Like I said, there is no KDE code involved in how Telegram draws its own system
tray icon. If Telegram itself is not at fault, the only other thing it could be
is that you have installed an icon theme or a Plasma theme that overrides the
standard icon with one that does not show a counter. Either way, a downstream
issue. Nothing KDE can fix here, sorry.
