I believe all these problems are caused by not saving and/or restoring display settings correctly. Would it make sense to create one, main bug titled something like "Screens/windows/panels/widgets arrangement not restored after login, reboot, wakeup, connecting or disconnecting a display"? Probably with feedback only from systems with Plasma 5.7.0 or newer.
