(It is possible it's happened previously, but I'm not normally physically near the machine during a session so might not notice). The only possible triggering action I can think of from that session was me toggling fullscreen in TigerVNC, which is set to resize the remote desktop to client window size (using xrandr - this would presumably happen on initial connection too as I don't have initial screen res set to match the client display I use).
