I have to admit that I haven't pin-point the root cause of this. 
It happens completely randomly but it's quite often. The only solution I have
found so far is to restart the PC.

What is happening: 
Icons in the task manager flicker between the active and the previously active
window. 

It starts randomly. I thought it was caused by Chrome, but then I realized that
is happening with Firefox as well. Also, some times it stops, when I select an
application (either via the Alt-Tab, or via the Task Manager). 

I am suspecting a few things but I would prefer to explorer some options as
well as a strategy to identify the possible root cause with someone what knows
better the underline technology. 

STEPS TO REPRODUCE
1. No idea. I have no idea how it started or how to re-produce it, or why it
stops or how it stops!

EXPECTED RESULT
Task Manager should only show the active window.

Task manager is configured as follows: 

Show Tooltips
Highlight windows when hovering over task tooltips
Mark Applications that play audio
2 rows max
Group by program Name
Cycles through tasks 
Combine into a single button
Group only when the task manager is full 
Short: Manually
Keep launchers separate
Minimizes on click, 
Open a new window with middle click
Show only tasks from current Activity
Unhide when Window wants attention 
New to the right.
