I ran into this myself before finding this bug report.

STEPS TO REPRODUCE
1. Add a Quick Launch widget to the desktop
2. Add one or more icons to it. The ones I added:
- A shortcut .desktop file in ~/name.desktop (A Firefox profile launcher)
- Two shortcuts to applications (Telegram, KWrite)
3. Reboot

OBSERVED RESULT
Telegram and KWrite are missing from the Quick Launcher
The widget has only one icon, for Firefox. The icon is the "gear" rather than
the Firefox icon.
Clicking the icon does not launch Firefox. There is an error "The file or
folder /home/tracey/Firefox Custom.desktop does not exist. "
That is not the name of the desktop file

EXPECTED RESULT
All added icons are still in the task manager

ADDITIONAL INFORMATION
Related, I had also pinned Telegram to the task manager after launching it.
After rebooting, the Telegram icon was no longer pinned.

A Quick Launch widget that I had created months ago doesn't have this problem.
The icons stay the same after rebooting.

Due to other issues, I created a new Quick Launch panel, logged out of Plasma
and logged back in. The panel had saved the icons properly. After a reboot, the
icons were still present.
It looks like there may be something about rebooting that's different than not
logging out that causes the improper save.

I will also note that a few of the times I saw the problem, I had to hard boot
the system because of an nVidia driver problem causing SDDM to crash on wake
after sleep.
