For Ubuntu/Kubuntu we follow the Ubuntu Doc Style guide that is in svn or available at doc.ubuntu.com. The document is saved as .xml and then built into HTML through a Makefile. The kubuntu-docs package includes these documents that are then viewed in KHelpCenter
