Menyerahkan pas foto ukuran:
2 x 3 = 5 lembar berwarna (baju putih + dasi, layer belakang biru)
3 x 4 = 5 lembar berwarna (baju putih + dasi, layer belakang biru )
3 x 4 = 5 lembar hitam putih (baju putih + dasi)
Fotocopy Ijazah SD/SMP ( kelulusan tahun 2010 )
Foto Copy 5 lembar (jika ada yang telah dilegalisir sekolah).
Foto Copy Akta kelahiran ( khusus paket A )
