Booting a 12.04 LiveCD enumerates the disks correctly. Booting a 16.04 Lice CD enumarates the disks incorrectly as described above. I'm pointing the blame for this at mpt3sas.ko (kernel module loaded for the LSI hba) which was previously mpt2sas.ko This bug is completely repeatable on every boot.
