You could combine pads elements (as many as required but give each element of the composite the same pad number), or you could probably use just use a rectangle or an oval. The former will give you the shape you want, but the disadvantage is you will have to hook together the elements with track after you place the footprint. However, since (I think) you would only need a circle and a rectangle, that wont be a big problem.
Does anyone know if there is a reason why these "awkward" pad shapes are being used in modern package designs?
