Main menu 
Dimensions > User Grid Size

Select mm
Set X and Y to 1.0
OK

--- or ---

select inches
set X and Y to 0.0394
OK


In main screen, use the grid select drop down (just under the rubbish bin
icon) and at the bottom of the list is user grid. select that and you
should be set.


Remember to use the rel co-ords at the bottom of the screen. (hit space to
zero them) also note that the mm or inches measurement unit icons on the
left hand toolbar apply to them so you can switch between units to check
things out.

There will be a slight error at times due to rounding, if I remember
correctly kicad uses imperial measurements internally and there is a
conversion done between them and metric.
