Could this also be an issue of variations in the headings' $q fields, which Koha tends to not display in search results, and when used there's no punctuation between the end of the $a and the beginning of the $q subfields? In your example, above, the headings could be for different people, and so should not be merged, e.g., could be: 100 1_ Smith, Alexander Gordon. 100 1_ Smith, Alexander Gordon $q (Alexander Gordon Frank) 100 1_ Smith, Jim $q (James Allen) 100 1_ Smith, Jim.
