In the situation where the "RenewalPeriodBase" syspref is set to "the old due date of the checkout" and an outstanding fine on it is paid off, the item would be renewed but, as you say, it may still be overdue if the old due date is further in the past than the renewal period. However, this is also the case if a regular renewal is carried out in the Circulation module, no warning is given in this situation, so an item can be renewed but still be overdue.
