No, the patrons checkout history only shows the time the three items
were checked out and no other details.
I realise it should not be possible to delete items/records that are
checked out but that certainly looks like what has happened.
Could there be a way to remove the issues at database level?

I am not sure what's going on there. If you look at the checkout history
for the patron, does the information there display correctly? (you might
have to turn on the 'intranetreadinghistory' system preference first)
In Koha it should not be possible to delete records/items while they are
checked out.

It looks as though biblio records have been deleted whilst items
checked out. The librarian suggests this may be true but it is unsure.
Given there are no details for the 'blank' checked out items, how does
one go about rectifying it?

Hi Client,
if you use the developer tools in your browser it might help to see a
more detailed error message.
One case we saw recently where the checkouts list would not load was
when the user had checked out an item without 245$a ($p was filled out,
so it was not obvious on first glance when looking at the record). Also
check the items have all required values, like itemtype, homebranch and
holdingbranch set.
