According to the documentation, there is a way to attach files to patrons :

Seems to be working since 23.11 :)
However there is no proper integration of e-signatures devices in Koha, nor an 
automated way to fill a pdf or form from the browser and have it attached to 
the patron.
That is quite a specific development, I think like David this should be done as 
a plugin if needed.
