Eh, most U.S. restaraunts have about the same level of cleanliness in my experience. My rule of thumb is to check the floor when I walk in. If they can't be bothered to clean the floor that customers see, odds are the kitchen isn't someplace I want preparing my food.
I caught part of a show a couple weeks ago... Dateline or something, where they were doing a return investigation into a place most don't think of.. public school cafeterias. There were some (they'd show the footage from the first time) that were so disgusting it was scary.
