Well you could do this with dia. It would be pretty
easy to write a program that would generate dia XML
from input like the data you show. See
http://www.gnome.org/projects/dia/

Another way to go would be to generate SVG. 
http://en.wikipedia.org/wiki/Scalable_Vector_Graphics

If I were doing this I would probably go for the SVG 
approach since that gets you to a representation
that is standardized and well supported right off the
bat. 

If you can spend a couple of bucks (ajbucks? :) then
you might want to check out

But if you really want to get down and dirty just
use Microsoft Word
