I've been hosting my mail and blog at Speakeasy for nearly 10 years and they're rock solid. I can add my own records to DNS, too. :) (A, CNAME and MX.) My rate ($14.95 a month) might be grandfathered, but I imagine a new hosting account wouldn't be much more. The only thing lacking (but I don't use it on that site) that you mention is the 'M' from LAMP. I think that's extra. They were recently purchased by Best Buy for some reason, but so far I haven't noticed any changes. Their tech support often knows more than I do, and I consider myself a moderate to high power user.

I have another website that I anticipated would maybe one day get some traffic, so I took Yahoo! up on an offer they had a few months ago for an unlimited storage, unlimited bandwidth business website. I think I pay 30 bucks every other month and I have to admit that they strike a very good balance between 'any idiot can make a website' and 'be a nerd if you need to'. As far as I can tell the only thing missing is shell access, but everything else is there including mail, ftp and MySql. I easily set up a Joomla! site and it's been running smoothly for a couple months (though I see very few visitors.) Their set-up is a lot of point and click, somewhat idiot-proof, but they provide some common tools like MyPhpAdmin for MySql, etc., if you poke around and look for them. I'm surprisingly satisfied with Yahoo! hosting.
