This should fix it (untested, but I'll test it soon):
choose-mirror (1.17ubuntu3) dapper; urgency=low
* If mirror validation fails but we're installing from a CD that includes
the base system, then set the suite and codename to reasonable defaults
and exit successfully (closes: Ubuntu #21309, #21091).
