From a brief glance, I would say that the user needs to install the breeze icon theme. The Kexi package in Kubuntu/Ubuntu is a direct, automatic import from Debian, and as such is not actually touched by Kubuntu or Ubuntu folks at all. As a so-called Universe (1) package, it has zero testing and only has support from the community - i.e. not officially supported. I think the user is hitting this bug (2) (3) I imagine it should be fairly simple to fix.
