Could be a few things, here's one that happens to me:

If some users have busy mailboxes, as I do, sometimes I have new
messages so I start to listen. The  first or second message is
important so I start to act on it right away and forget that there are
still more new messages that I did not listen to. The light goes off
and I don't get the message until I log in later, or if another
message comes in.

I believe you can turn on "notification for unreceived messages" to
help that issue.

This also happens to me a lot when I get my messages by cell
notification. I do not hear when the lady says how MANY messages there
are, but I just press 1 and start listening. Then I hang up after I
hear the first message, not realizing there were more in there.
