There are a couple ways to go.
I set up one customer so the supervisor could conference with any of the
operators. The supervisor had a headset with a mute switch, and a tape
recorder interfaced through the handset jack. There were only a few
operators. One drawback with this is that the operators could easily tell
that the supervisor was connected by looking at their phone.

The other way is to use a service observing unit such as the DEES CM-30 or
The Viking SO-24. They are not cheap which is why that one customer of mine
went with the conferencing.  I installed the Viking SO-24 and it worked well
for another customer.

There is a connection for one supervisor telephone, and connections for the
co lines. The supervisor can dial digits to select which line to monitor,
then press another specified digit to join the conversation, and another
digit to start a tape recorder.  The Viking unit handles up to 24 lines, the
DEES unit up to 30 (DEES does not have the recording feature, but you could
still connect a recorder to the supervisor phone).

You can also feed the tip/ring of the supervisor phone connection into an
unused co port on the ksu to allow monitoring from other phones on the
system.

We even set the Viking unit up with a cordless phone for the supervisor, so
he could walk around the call center and choose who to listen to while he
was standing near them.  There was a bit of a problem with the lower voltage
on the line needed for the cordless to operate. A hardwired phone worked
fine but the cordless phone needed an extra battery in the line.
