Several ideas,

pull the pair loose at both ends. 

check for high resistance (in megohms) from the pair
to ground.  If you get a reading, the cable is wet or
has creep paths somewhere.

make sure there are no "taps" to other cable

if there are blocks in a basement mounted on a
backboard, make sure they are dry and clean.  mold
will form a creep path in cool damp locations,
especially on plastic devices.

make sure all cable is pair twisted (like cat 3 or 5),
just a short piece of the old style "station D" cable
will add hum and crosstalk to a perfectly good
install.  Any of this r/gn/y/bk unpaired/untwisted
should be removed, this stuff is questionable for even
doorbell wire use ;-)

ultimate test, lay a piece of good cable from the
demarc to the ksu, see if the noise is still there, it
still could be a telco problem.  (this is a temp cable
layed point to point just to test, thru the doorway,
window etc)  If the noise is still there, it's not the
in-place cable.  a "ground loop" (ac voltage
differential in the power line grounds) will make the
hum appear when you connect a ksu, but won't show up
with a butt-in set.
