"Extremely rare" is right :) And, I always thought that "a Stanhope"
was some kind of a horse-pulled vehicle, but it seems to mean "a
spangle" here?

A Stanhope as used here is a little bitty microscopic viewer that shows a picture of some kind - often a public place or famous building, for example. The technology for making these arose in the mid-1800s and you can find them inserted into all kinds of things - a quick web search brought up letter openers, needle cases, etc. All you need is space for a hole and a 3 mm x 8 mm glass rod on which the picture appears. There was quite a craze for them in the late 1800s and well into the 1900s.
