Someone on the list here gave me a tip a couple years ago which I have
used several times and it seems to work very well.  Use a 2x4 or any
chunk of construction wood and drill a perpendicular hole through it. 
Trim it down to the size you need and use the hole as a guide.  Needless
to say, you may have to discard it after a couple tough holes as the
guide hole will widen, but it works great.
