* rewrite Section 1. The program development cycle in a nutshell to
correct server starting description, correct paths, update screenshot
* remove redundant section 2.7. Debugging, link to Debugging chapter
instead. (The Debugging chapter still needs a rewrite: see LPP-9661)
* remove redundant compiler info: link to Chapter 49
Chapter 49. Understanding Compilation
* section 1.1. The Developer's Console: remove redundant and outdated
descriptions; link to appropriate section in chapter 4
/examples/hello.lzx: just give the canvas a height for the screenshot
all: change Developer's Console and Developer Console to Developer Toolbar
remove outdated screenshots
add screenshot for Developer Toolbar
