Details:
Remove the IDE specific set up info from the dguide, and just point to
the Wiki in the overview.
Fix DocBook markup problems
NOTE: Some information in this chapter is known to be wrong. In
particular, the stuff about the DTD conflicts with the current
reality, because we don't ship lzx.dtd with the distribution. See
the blocking issue.
