Shouldn't (if the OS is a Mac Osx based) the actual Mac Osx widgesets be
enough?

This is possible, but I highly doubt it based on previous experience.
Windows CE for example is a small version of windows, but it´s very
different. Linux for ARM is very similar to normal linux, but however
there are some minor things to be thougth about, like many times there
is no X11, only framebuffer, and other differences.

So it all comes to how much is this small os similar or different from os x.

What are the possibilities for lazarus in that task?

Without more information about what exactly iPhone is, it´s hard to
tell. But everything is possible, of course.
