I just came accross this thread, but I have been thinking of a couple of examples. I currently use Visio for this type of design, but I'll see if I can move it over to Dia.
Are you looking for basic to advanced type setups, most up mine have been involving strictly IPSec type setups, with no internal servers. I do have one internal server with public access. I can make a digram of that surely. heh. Yes a bit-mapped format (jpeg/png) and a vector format for source, so we can manipulate the diagrams. I think dxf is the only widely supported format. W3C SVG is to new.
