It's been awhile since I looked at the number on the chip, but I
think my ISA-to-PCMCIA bridge is a Vadem VG-469.

  # cat /proc/bus/pccard/0*/info

I use the following values in /etc/default/pcmcia (on Bering):

  # cat /etc/default/pcmcia     
  PCMCIA=yes
  PCIC=i82365
  PCIC_OPTS=i365_base=0x3e2
  #PCIC_OPTS=
  CORE_OPTS=
  CARDMGR_OPTS=

IIRC, I had some problems with it until I got the PCIC_OPTS value
set properly.  Not sure how similar the 468 and 469 are, but maybe
that will help.  Here's a snippet from the console during PCMCIA
initialization:
