Hmm, I can't test this as it seems that on our site it takes the "other"
path through that part of the code.  Does it say what line this error occurs
on?

What steps will reproduce the problem?
1. New user sign up
2. Admin login and approve
3. Approve account as player

What is the expected output? What do you see instead?
Couldn't save: HY000 2014 Cannot execute queries while other unbuffered
queries are active. Consider using PDOStatement::fetchAll(). Alternatively,
if your code is only ever going to run against mysql, you may enable query
buffering by setting the PDO::MYSQL_ATTR_USE_BUFFERED_QUERY attribute.

What version of the product are you using? On what operating system?
2.6 latest snapshot


Please provide any additional information below.
I posted the last dbh->sth error and after that fix, this is the next error
showing.
