We've had a couple of reports like that here too, and they have turned out to be user error. Specifically, captains are forgetting to click "submit" on the final confirmation page. If you have access to the raw server logs, you can look for lines with /game/submitscore/xxx/yyy in the URL, where xxx is the game id and yyy is the id of the team submitting the score. Depending on what options you have enabled, you should see a consistent number of POST records, and if they forget that last click you'll see one less. Greg.
