I suspect it's always been like this but few people would notice
because people generally don't mix cases.

FWIW, I strongly believe that the query interface should be case
insensitive (I type lower case account names for 'reg' and 'bal' all
the time, even though I use "proper" casing in my ledger files and I
suspect it's the same with a lot of users.)

So I guess in order to be consistent, ledger should consider
Expenses:Groceries and Expenses:groceries the same.
